ABSTRACT

Cardiovascular disease is a class of diseases involving the heart or blood vessels, which accounts for about one-third of all deaths worldwide each year. Unhealthy diet, lack of physical activity, smoking and excessive alcohol consumption are all risk factors for cardiovascular disease. With the increasing number of night shift workers, the number of patients with cardiovascular disease has increased, and night shift work has gradually become a risk factor of cardiovascular disease. At present, the mechanism of cardiovascular disease caused by night shift work is still unclear. This review summarizes the relationship between night shift work and cardiovascular disease and its related biochemical indicators, and discusses the research on related mechanisms.

ABSTRACT

This systematic review aimed to evaluate the association between fixed night work and overweight or weight gain. PubMed and EMBASE were searched until October 2021 for studies evaluating the association between fixed night work patterns and the risk of overweight or obesity (for cross-sectional designs) or weight gain (for longitudinal designs). The outcomes extracted were mean body mass index (BMI), mean BMI difference, overweight %, obesity %, odds ratio, relative risk, and prevalence ratio. The quality of the report was evaluated using the Strengthening the Reporting of Observational Studies in Epidemiology (STROBE) checklist. The protocol was registered at PROSPERO (# 42017080515). In total, 25 studies met the inclusion criteria. All studies were observational, 16 were cross-sectional, three were cohorts (two also had baseline cross-sectional data), and the other eight were cross-sectional at baseline and had longitudinal follow-up studies (six prospective cohorts, one retrospective, one interventional). Most had good reporting quality. The fixed night work definition and weight outcomes varied according to the different studies. Most of them found an association between fixed night work and overweight, obesity, or weight gain. This systematic review reinforces the evidence that fixed night work is associated with overweight or obesity, and prolonged night work exposure leads to weight gain. Future research should be conducted with more accurate measures and a prospective design. Fixed night workers should be monitored and advised, especially those with prolonged exposure.

ABSTRACT

Introduction: One in seven hospitalized patients experiences an adverse event related to administration of medication. Medication errors are one of the most important causes of preventable mortality and morbidity. Objective: To assess the efficacy of a training intervention with the population of night shift nurses in an acute care hospital, in order to improve compliance with the protocol for the safe administration of medication. Methods: Experimental trial, pre-post training intervention, carried out at Hospital Clínic of Barcelona, during 2015-2016. The population consisted of 268 nurses in two night shifts. The sample consisted of 177 participants (88 from the control group and 89 from the experimental group). The intervention consisted in information sessions and access to a written procedure. The measurement instrument was the Institution's Standard Work Procedure by means of a compliance check-list. Uni-bivariate study was performed, using chi-square and Fisher's test with a significance of P < 0.05. Results: 219 observations were carried out in the control group and 207, in the experimental group. Of seventeen variables analyzed, only three showed significant differences: in the experimental group, knowledge of the procedure improved, increase in the use of soap and water over hydroalcoholic solution, and worsening of standardized identification of drugs pending from being administered. None of the fourteen remaining variables showed significant differences. Of 426 observations, only three medication errors occurred in the control group, corrected before its administration, and zero occurred in the experimental group. Conclusions: Classic training interventions with passive receptors may not be effective to improve nursing practice in safe administration of medication(AU)

ABSTRACT

Introduction: To guarantee 24 hours of care in hospital institutions, the existence of nursing teams is essential whose activity is carried out at night. There are organizational factors that provoke, among night teams, the perception of fewer opportunities for professional development and less institutional attention. This can have an impact on the quality of care and even on the health of professionals. Leader nurses are responsible for creating and maintaining a positively influencing work environment. Objective: To evaluate a nursing management project through the impact on the perception about the work environment on the night shift. Methods: Pre-post quasi-experimental study carried out with intervention on the population. From a population of night-shift nurses (n=268), we obtained a sample of 268. The instrument "Practice Environment Scale of the Nursing Work Index" was used, which allows evaluating and comparing factors from the nursing practice setting. Univariate statistics of central tendency and dispersion were determined, as well as contrast statistics with significance p < 0.05. Results: In 41.94 percent of the questions, significant differences were obtained, highlighting factor 3 (nurses' skill, leadership and support by their heads). The global mean value obtained in the PRE test was 2.37 (95 percent CI, 2.22-2.52) and the global mean for the POST value was 2.49 (95 percent CI, 2.34-2.95), with p=0.0254. Conclusions: A nursing management project's evaluation through the impact on the perception about the work environment in the night shift shows improvements with significant differences(AU)

ABSTRACT

ABSTRACT Objective Different pathways may lead from night work to metabolic diseases, including type 2 diabetes. This study aimed to explore the direct and indirect pathways from night work to glycemic levels, considering the role of physical activity, waist circumference and snacking using data from ELSA-Brasil. Materials and methods A structural equation model was used to confirm the pathways from night work to glycemic levels. The latent variable, "glycemic levels", included fasting glucose, glycated hemoglobin and 2-hour plasma glucose. Results A total of 10.396 participants were included in the analyses. The final model showed that among women, night work was associated with increased glycemic levels. A statistical significant association between night work and glycemic levels mediated by waist circumference was observed among women and men. Conclusions The association between night shift and glycemic levels can be interpreted as an important step toward understanding the pathways that could explain night work as a risk factor for diabetes using epidemiological data.

ABSTRACT

Introduction: The study of social representations was undertaken to explore the conceptions that musicians have about health, illness and care, and the way they relate such aspects to their musical practice. Objective: To understand the social representations of health, illness and cultural care of traditional musicians. Methods: Descriptive, qualitative study, based on the ethnographic method, which combined non-participant observation with a semi-structured interview conducted on 15 traditional musicians in the city of Santa Marta, Colombia, during the years 2015 and 2016. Results: The social representations of traditional musicians allow to generate an approach to how they think about their health in relation to the musical practice and their context. The awareness that musicians have of it revealed that elements such as night work, consumption of psychoactive substances and situations derived from the musical performance generate disorder to their health. They were found to develop care strategies associated to a preventive and curative attitude based on traditional methods. Conclusions: Social representations constitute a basic resource to understand the relationship between cultural dynamics and health-disease processes. The contrast with the available scientific literature revealed that, despite constituting different modalities and contexts, there is the possibility of finding similar alterations that may affect the health of these people(AU)

ABSTRACT

Evidence suggests association between labor characteristics such as night shift work, uninterrupted shift and being active at work with weight and adiposity outcomes. Objective: To analyze the association between work characteristics and weight and adiposity outcomes in the Brazilian population. Methodology: A cross-sectional and population-based study with data from the Pesquisa Nacional de Saúde (PNS) of 2013. The sample consisted of 36.247 Brazilians workers with 18 years old or more, who had weight, height and waist circumference measured, with exclusion of women who reported being pregnant or could not tell if they were pregnant. The outcome variables were overweight BMI ≥ 25 kg/m2, obesity BMI ≥ 30 kg/m2 and abdominal obesity (≥ 102 cm in men and ≥ 88 cm in women). Night work, uninterrupted shift work and being active at work were investigated as explanatory variables. Sociodemographic and lifestyle characteristics and years in the main work were analyzed as descriptive and adjustment variables. The analyzes were performed with help of the software Stata 14.0, considering the weights of the research sample design. Poisson regression models with robust variance were performed to estimate the association between main exposures and outcomes, considering the statistical significance level of 5%. Results: After adjusts, individuals who work at night and work in uninterrupted shifts respectively, presented a higher prevalence ratio for overweight (PR: 1.07 CI 95% 1.03-1.12; PR: 1.09 CI 95% 1.00-1.20), obesity (PR: 1.18 CI 95% 1.061.31; PR: 1.29 CI 95% 1.02-1.63) and abdominal obesity (RP: 1.12 IC95% 1.04-1.21; RP: 1.25 IC95% 1.06-1.48) when compared to those who work at conventional times. Being active at work was inversely associated with excess weight outcomes (PR: 0.92 IC95% 0.88-0.96), obesity (PR: 0.83 IC95% 0.75-0.92) and abdominal obesity (PR: 0.86 IC95% 0.81-0.92) in relation to those inactive at work. Final considerations: The results evidenced that night work, uninterrupted shift and being active at work may be potential risk factors for weight and adiposity outcomes.(AU)

ABSTRACT

Context: Obesity is a disease of epidemic proportions worldwide. This condition leads to reduced quality of life and life expectancy, functional disability, and mortality. With multifactorial etiology, it is the result of genetic interactions, lifestyle, and environmental factors, such as labor. In a society that operates 24 hours, there is a constant increase in night-shift workers. The metabolic changes caused by night work lead to problems such as an increase in excess body weight. Objectives: To verify the occurrence of factors associated with overweight among employees who work in shifts in a chemical company. Methods: This is a cross-sectional study carried out at a chemical company in the city of São Paulo, in 2013. Twenty-nine workers from the production department (76.3% of the total) agreed to participate in the study and were divided into three groups, according to their work schedules. An interview was conducted for the completion of questionnaires and physical evaluation. A case was categorized as overweight when the body mass index (BMI) was ≥25 kg/m2. Statistical analyzes were performed between the outcome and the variables using Fisher's exact test. Results: The group was composed of all male workers, with mean age of 40.2 years (SD±6.53) and average of 10 years (SD±8) working in the company. Of the total participants, 93% did not smoke, 62% did not drink alcohol, 41% were sedentary, and 69% worked the daytime shift. Overweight was present in 82.8% of the workers. There was no statistical association of overweight with shifts or other covariates. Conclusion: It can be concluded that there is a high prevalence of overweight in the studied group. The work shift and other covariables did not show statistical association with overweight among the workers in this research. However, it indicates that this company need to create programs of integral attention to the workers' health aimed at overweight reduction.

ABSTRACT

ABSTRACT Objective: This study was carried out to determine the nutritional status of shift-working female nurses at a university hospital in Ankara, Turkey. Methods: A total of 110 volunteer female nurses (n=56 control group, n=54 study group) were included in the study. A questionnaire with a three day food record collected the study data. Results: The mean daily energy intake of the study group was higher than that of the control group (1756±659 kcal versus 1694±431 kcal, p>0.05). While the carbohydrate intake (196.3±85.5 g versus 185.9±54.7 g) and fat intake (79.5±29.5 g versus 77.1±22.6 g) were higher in the study group, the protein intake was higher in the control group (59.4±17.6 g versus 57.6±21.6 g). The mean iron intake was statistically higher in the control group (10.6±2.9 mg versus 10.0±4.0 mg, p<0.05). Conclusion: To improve night shift workers' performance and nutritional status it is important to provide accessible, healthy, and quality food services.
